Separate medical and prescription drug plans with different premiums apply for employees in Hawaii and Puerto Rico, and expatriates on international assignment.
Employees who use tobacco during the three months prior to making benefit elections are subject to the tobacco user surcharge, including employees eligible for the BeneFlex Medical Plan in Hawaii and Puerto Rico. The surcharge also applies to expatriates on international assignment, who have access to complete the Viverae telephone coaching sessions and the Breaking Free from Tobacco online program, or who may complete and submit the International Tobacco Surcharge Waiver Form to earn the surcharge waiver.
